Â .Evaluation of the Alvarado score in comparison with the Bedside Index for Stroke Severity (BIS) score for evaluating acute stroke severity using a computer database of patients.
The Bedside Index for Stroke Severity (BIS) Score is a new standardized clinical tool to assess the severity of stroke in the acute phase. The aim of this study was to compare the performance of the BIS Score to the clinical score devised by Alvarado et al. (the Alvarado Score) in a large sample of patients using a computerized database. All acute stroke admissions to a large teaching hospital in Sydney, Australia between June 1998 and November 1998 were entered into a computerized database that had been specifically designed for stroke management and research purposes. The database contained data on patients’ demographics, stroke severity, prior vascular risk factors and outcomes. The initial stroke severity was determined by the Bedside Index for Stroke Severity (BIS) Score on admission. Patients were assessed by stroke physicians using the Alvarado Score during the same admission. In total, 631 patients were entered into the database and 454 were available for study analysis. The mean age was 69+/-13 years and 192 patients (40%) were female. There were 201 patients (44%) that had a BIS Score of 3 or less and 114 patients (24%) were classified as having a mild stroke on admission. On admission, the mean BIS Score was 4.5+/-3.6. Four hundred and nine patients (94%) had an Alvarado Score of less than 7. Forty-one patients (10%) had a BIS Score of 3 or less and were classified as having a mild stroke on admission, whereas 11 patients (2%) had an Alvarado Score of 7 or more. Patients with a moderate to severe stroke on admission were more likely to have a lower mean BIS Score (3.9+/-3
